How to become a patient
The Community Free Clinic of Huntsville serves residents of Madison County who do not have health insurance and are not covered by Medicaid or Medicare.
Eligibility is verified using two forms of identification, including one with a current photograph, along with verification of income.

Please bring the following when registering as a new patient:
• Valid photo ID
• Proof of income
Register in Person
New patient registration takes place in person on Tuesdays and Thursdays from 2:00 PM.
